- The name and position of the person or the name of the office.
- When and where it happened, and whether it has happened before.
- Whether anyone has tried to complain, and what happened.

What happens next
Every report is read by a person. We assess it, check it against evidence, and decide what to do, as set out in our investigation methodology. We cannot investigate every report or promise a result, but we read all of them. Your identity is protected under our source protection policy, and we handle your details as described in our privacy policy.
